Faculty and other responsible university officials should use this form to report allegations of academic misconduct, regardless of resolution method.
Report General and/or Substance Use Misconduct
Campus and community members may use this form to report allegations of general and/or substance use misconduct. Behaviors that may be reported through this form include, but are not limited to, alcohol use, disorderly conduct, controlled substances, harassment, physical violence, and violation of other university policies.
Campus and community members may use this form to report suspicion and/or allegation of hazing against registered and recognized University of Oregon student organizations.
Report Discriminatory Misconduct
Campus and community members may use this form to report allegations of discriminatory misconduct, which includes incidents of discrimination or harassment based on a protected class, sexual assault, sexual harassment, dating or domestic violence, gender-based harassment or bullying, and/or stalking.
